The success of the work we do hinges on building successful relationships within your organization. Care coordination

We work with you to design and implement your Care Coordination Program:

• Annual Wellness Visits (AWV)

• Chronic Care Management (CCM)

• Behavioral Health Integration (BHI)

• Transitional Care Management (TCM)

Based on your program needs, we will employ Care Coaches, to support the patients in reaching their goals while maintaining the Centers for Medicare & Medicaid Services (CMS) programmatic requirements.

Remote patient monitoring (RPM)

Remote patient monitoring (RPM) uses digital devices to collect various forms of health data from your patients and electronically and securely transmits that information to the health care providers for assessment and recommendations.  RPM is designed to allow you to continuously track healthcare data for your patients, keeping them healthier and ultimately reducing readmission rates.  

Our monitoring platform allows the collection of health data like vital signs, weight, blood pressure, blood sugar, blood oxygen levels, heart rate, and electrocardiograms. In addition to the platform, we can also provide connected devices, patient support and any necessary follow-up.

Telehealth

By leveraging our telehealth platform, we help you provide remote care to your patients. We help design the process for setting up and managing appointments, provide the infrastructure to accommodate the virtual visits, and maintain compliance with any changing policies pertaining to billing and reimbursement.
